Consent to Enter

I/We wish to have the above maintenance or repair attended to as soon as possible, and consent to the Landlord, superintendent, their agent or their service agency entering the rental unit today between the time of submitting this Request for Maintenance and 5:00 P.M. to inspect and/or complete the maintenance requested. This shall be considered your Consent to enter the unit in accordance with Section 26 of the Residential Tenancies Act of Ontario.

If the Landlord is unable to enter the same day as your request, you consent to the Landlord, superintendent their agent or service agency to enter your rental unit within the following 2 business days following the submission of this maintenance request without further notice to you however prior to entry, the party requiring entry shall attempt to contact you and/or knock before entering the rental unit.

Should the maintenance requested not be able to be completed as noted above, I consent to the Landlord providing me formal Notice to Enter by email to the address provided. The 24-Hour Notice to Enter shall be provided in accordance with Section 27 of the Residential Tenancies Act of Ontario. A copy of which may be placed in your mailbox.

You confirm and understand that should you or any occupant inside your unit or that has visited your unit in the last 7 days had or has shown any signs or symptoms of any illness, you shall disclose that to the Landlord, superintendent their agent or service agency prior to them entering your rental unit.

I acknowledge that I am submitting this maintenance request electronically and as such I am executing and submitting this request as if I had done so in writing and that I authorize the Landlord and/or their agent to act upon this request.

If you are not able to consent to all the above Acknowledgments indicated above; please attend at the Administrative Office and complete a manual Maintenance Request Form so we may determine how to best proceed with fulfilling your maintenance request.

Prior to attending at the administrative office, you must contact the office by telephone at 613-962-4117 and arrange an appointment to complete the proper paperwork.
